Don't use DHL to ship to Portugal (I bought a 157€ product and payed 135€ of taxes because DHL decided to put in the customs formulary that they charge around 117€ to transport to Portugal and so the Customs in Portugal have billed me (with the dhl services) the 135€
I have bought 157€ product with free delivery and because of DHL just cost me more 135€....
Please use a better company, or ctt

The problem is that finding this or other similar forms in pdf is very difficult and it takes a lot of time to create a file yourself. Moreover, if you do not have a program for working with pdf files, then you will definitely suffer. I spent a lot of time on the internet and ended up finding a loan form 8962 in pdf format. You can download it on this site https://pdfliner.com/form_8962 . You can't even imagine how happy I was because many sites didn't have any samples. So you don't have to thank :)
